If the tile isn't broken, just deep clean it, and restore the tile. If you have to replace (and if it is not broken, don't fix it!), replace with black and white hex, or penny round. For a teenage boy, I would do white everything else (easy to bleach clean/looks crisp against the green). I guess it would be a mix of his style, your house style. Bathrooms get wet (and moldy) even without teenage boys, so I wouldn't do a cloth curtain that you need to remove and wash unless you can count on him to do it. Maybe a reproduction of a vintage travel poster for a place he likes or a vintage car from etsy that you can put in cheap IKEA wood frames.
