I would first see how many embryos you get and go from there. What is your age and what clinic are you at? SGF generally recommends eSET.

I have IVF twins. They are about 10 - so it was before CGH was common. Given my age at the time (38), 2 was the recommended # of embryos to transfer. I had healthy twins at 37+ weeks with no NICU time. I did have gestational diabetes but no bedrest or other issues (worked right up until they were born). So a darn near perfect outcome BUT I would never ever go along with the decision to transfer 2 again

It is a very stressful pregnancy with dozens and dozens of doctors appointments. The doctors are on-edge, continually predicting disaster. Then I needed a csection because baby B was breech. And infant twins are a handful - I essentially don’t remember their first year. It was rough on our marriage. Getting two infant daycare spots is basically impossible, so you will likely need a nanny, which is $$$. After the misery of IF, the pregnancy and their first year then felt like all-stress-and-no-joy. It was a rough few years. And having 12 pounds of babies messed up my body - I have bad disastasis and have had back pain since they were born. Also, I get asked A LOT by strangers if I did IVF when they figure out our kids are twins - and I have found that it annoys me when I am asked about the functionality of my reproductive system by the cashier at Target, a random mom at the pool, etc.

There is no doubt that twins are efficient & cute, but knowing what I do now, I’d do a SET - especially since you can identify genetically normal embryos now-a-days using CGH.

I would definitely recommend transferring 1 on your first try. I transferred 1 each time for 4 cycles. The 5th cycle we did co-culture and transferred all 3 viable blasts (recommended by RE because of so many previous failures). We ended up with twins. My kids were born at 29 weeks, and I had life-threatening pregnancy complications. I would transfer one at a time if I could do it again.

Your age is also relevant, both in terms of multiples and pregnancy risks.

Would be cautious of advice on this via DCUM or friends. The evidence, research and recommendations have shifted dramatically over the years. Talk to your doctor for recommendations.

Generally the doctors recommend singleton.
