I don't think it's any worse (or better) than calling someone a 'bitch' - but you don't call women a bitch during sex, do you? At least not without going over it with the person ahead of time, and with some role-playing or something.
No, but he's not calling her a c. He's using the word to refer to her genitalia. Do you use clinical terms for your husband's genitalia during sex, then?
