This is pretty much what I was going to say. I would probably hate an unflattering picture of myself (which is most pictures, to be honest) and wish you hadn't sent it, but if you write a lovely note with it and say how much you love the picture and how you can see their connection with each other and how happy that makes you as a mother, than that might soften the blow for me.

+1 If it were sent to me would try very hard to see past the bottom and focus on the expressions. Might not frame it for the living room, but I'd love having it.

Send it. She knows she has extra pounds, no picture will hide that. What matters here is the bond that they share

This is what I would do. Especially if it's just one part (her bottom) that looks a bit wide. I think it's an easy fix.

Crop it as much as you can to emphasize the focus on their expressions. I'm an art director and most people don't crop anywhere near enough. Crop the top to just above their heads, the bottom up to the keyboard/hands, and most importantly, crop at least half their bodies off the sides. Then include a note about how much you loved their expressions as others have suggested.

If she makes comments about her weight gain, I wouldn't send it. She may be upset and ask you not to take pictures again. I have several elderly people in the family who hate having pictures taken - and they are generally not vain.
