Usually women don't even realize it until there's a smell and/or infection. Sometimes even then they don't put two-and-two together to realize it's a tampon left up there and are informed of such by their doctor.
One time I sat on the toilet and did a little straining and plop, a tampon fell out. I had completely forgotten to remove the end of period/last tampon and forgot about it for probably weeks. Never felt any problems, no smell, no cramping. Just, oh hello surprise tampon.
