+2 Don't use the microwave to heat the food before putting in the thermos. Heat it up on the stove. I did this many years for my DD and she said the food always stayed warm. Cold food, I would use an ice pack or add another thermos but this time fill it with ice cubes and put in freezer to keep nice and cold before putting cold item inside (like potato or macaroni salad - anything with mayo).
