How is your relationship with your cousin? I'd run interference with cousin first, but don't back down and make yourself sleep on a smoky mattress. If I drove 4 hours just to bring you a piece of furniture, I'd be annoyed, maybe, but some of your objections (size, smoke, lack of frame) are v reasonable and any reasonable person will not be offended by them.
Please do not mention that you like things to be matchy-matchy. That's something you should have considered before she schlepped the stuff 200+ miles for you, and it does make you look like an entitled jerk. But you have reasonable reasons for not wanting it.
Please don't buy a bed on Craigslist. Gross. Bedbugs. Ick. As a pp said, you can get a twin set from a discount mattress place for $50 bucks, regularly. It may not match, but who cares. They usually throw in a metal frame free.
This. If I offered you a free bed, and drove eight hours total to give it to you, and you told me later you didn't want it because it didn't match your other furniture or offended you sense of aesthetics, I would be pissed and think you were an entitled brat. If you said that you couldn't use it because the box spring didn't fit up the stairs so you couldn't use the frame--totally reasonable.
