I assumed my cystic acne was hormonal, too. I never had it until after I had my first child, and I assumed that they were caused by being off the pill and just general hormonal changes. I had them for several years. About a year ago I started eating much worse than normal for me for about 6 months, and strangely my face cleared up in that time. Several months ago, I went back to my typical healthy diet, which, for me, usually included eating yogurt and drinking some milk every day. A week or so later, the cystic acne gradually appeared. I stopped eating the yogurt and eating dairy daily, and my skin cleared up and I haven't had a cystic pimple since. I otherwise had never noticed any reaction to dairy (no stomach or intestinal issues), but I'm so glad I finally figured it out and got rid of the cystic acne.

Try oral spironolactone. It prevents receptors in the skin from binding with testosterone--or something like that. I used it for a while and it was great. Clears acne and gives the skin a nice glow. Only for women, though.
