Here is what I would want to know before making an offer. This information is necessary because I'd want to make sure the person would accept my offer.

Current Salary
Current Bonus Potential
Current Vacation/Sick
Current 401(k) match
Employee Profit Sharing amount

I've always recruited in competitive industries and often this information would be helpful in putting our best offer on the table.

Yeah I usually don't fill in my SSN on any forms- and if they REALLY need it they'll ask.

The Feds they will do that sometimes to match your current pay.
