For the woman who has everything I think a "coupon book" is a great idea. All you need is a printer and some imagination. Just create a book offering to do/pay for a few things that he normally does:
1. Trip to spa
2. Household chore
3. Go down on her.
I would not get pictures of the kids. They are hers, she knows them and she know what they look like - make it all about her.
The gift is not the book, its what in it. My DH got me one for valentines day one year (it was store bought). It had coupons for things like foot massages, dinner out etc. I would rip out a page give it to him and he would so whatever was on the coupon. Its not that complicated.
