Why do you care? You’re just as ill-mannered as MIL.
Are you one of those over sensitive people who won’t admit to weight loss drugs? Everyone is talking about those people behind their backs. If it was me, I’d rather someone just ask.
Oh yeah it's a great idea to confide in the type of people who talk behind your back.

How often do you really look a person up and down?
Sounds like you are looking for a reason to be offended.
Some people live life looking to be offended.
Take the compliment OP, offer some advice, and move on.
It’s not always a compliment, idiot. MIL went on and on to a person at church “complimenting” them, when finally the person shut her down by saying, “I have cancer, and I would give anything to have that weight and my energy back.”
Don’t. Comment. On. People’s. Bodies.
Not when it comes to bodies.
Don’t comment on people’s bodies! If you say “It’s great to see you” or “you look great,” they’ll open the door if thy want to.
Don’t. Comment. On. People’s. Bodies.
There is a huge difference between strangers at church and family.

Yes. But family still doesn’t get to demand medical info. You’re rude

Today, my mother in law, who I last saw at Christmas (and mind you, I have only lost 4lbs since Christmas) looked me up and down and said "Have you lost a TON of weight?". I said "yes" and she promptly asked me how. No compliment in there or anything. Don't do that to people, folks. It's incredibly rude. Just don't talk about weight. Ever.
Is she also trying to lose weight? Is anyone else in the family fat?
It doesn't matter. It's always rude to ask someone about their weight.

You don't know what's going on with others - just don't comment on their bodies at all.

Asking someone if they've lost weight or noticing that someone has lost weight is NOT a compliment or something to be flattered by. It's just an observation. And looking someone up and down in an obvious manner is just rude.
MIL could have just said "You look great" and left it at that.
