I return at Whole Foods, which does not require original packaging. The return machine provides a plastic bag, and prints a return sticker based on the QR code you present. And I've never once had a mix-up, even though I've returned many items that were part of a larger order and looked similar to other items in that order.

This happened to me recently. I bought four pairs of pants and returned 2. I selected the correct pants during the online return process, the Amazon emails I received with QR codes showed the correct pants, and I returned the correct pants. I was quite surprised to find that somehow they refunded me for the wrong pants (the less expensive ones!) and sent me emails that they were still awaiting my return.

I thought it was pretty fishy, and it was a real pain to sort out.

Op are you taking things out of boxes, using them etc before returning? If stuff is in original packaging with bar codes, it doesn’t make sense for this mistake to happen - because everything is just automated, computer register the stuff coming in and out. But if you’ve taken stuff out of boxes or not in original packaging, then I could see the switch issue happening.

My son periodically swears off Amazon and cancels his business prime, and then later he will open it up again. Huge issue with delivery drivers who claim nobody was at the business (they have an access code for weekends). sometimes they will say they left the item "with reception" and it will turn out they dropped it off at the bar next door.

Last time it was a part needed for a vehicle repair. They said it was delivered but no sign of it. Driver said left with reception. Replacement shipped. Then found it wedged between the back leg of an outdoor bench and the building by one of the building people who happened to drop their keys and saw the package when then bent down to retrieve them.

I have gotten more packages than I can count addressed to addresses I know do not exist. I have no idea how this happens.

Nobody can tell you what you messed up there, but shopping less should fix most of it.

Freaking stop returning everything to amazon!
