Mostly in this extreme case it's about 1) not using birth control and 2) very high fertility. In particular, that nursing doesn't stop (or potentially even really decrease) her fertility, which is fairly unusual. While nursing is not 100% effective as birth control, it definitely helps promote longer child spacing. Generally, a woman who nurses all her kids will have kids roughly 2 years apart (or more), but the reason they have so many kids is that she had one basically every year. Other people with their belief system (no birth control) tend to top out at more like 8-12 kids depending on age of marriage and fertility, and families like that are a dime a dozen in fundamentalist circles.
