We took her to the hospital right next to Westminster bridge and they saw us in an hour and we didn’t pay a dime. Was that an odd one-off, and would I have to plan something related to health insurance prior to doing this?

ER services (A&E) are always free to everyone. Urgent Treatment Centres, like Urgent Care here, are also free at point of care and you don’t need an address so will in practice be free, though I think in theory you should be charged somehow. You should still have at least travel health insurance though, because if something happens that isn’t an emergency and you want access to better services, you’ll need to cover that

If the OP tells the immigration officer that they are there for 3 months of vacation and then spend the time working, they've lied to an immigration officer.

Maybe. But who would know or care? There are only a few places that would come after you for this, like Switzerland if you’re working for a Swiss company.

No. It's no different from a work trip to the UK for a few days or a week, which I've done aplenty.
