I’ve known people who were not eligible for the rehab center their doctor preferred because they were on Advantage - someone else with the same diagnosis on regular Medicare was approved.
There are Medicare Advantage HMOs and there are also PPOs that have out of network coverage.

My DH found a person who specializes in Medicare who helped him figure it out. His monthly premium is dependent on our income, which is still relatively high, but not nearly what I pay for mine (self-employed, not yet 65.)
He has A, B, D and I believe N for supplemental.
