You don’t even need a doctor’s note. Just don’t return the signed permission slip.
If you do want to send a note, send an email to the coordinator and say your child will not be attending the trip. You don’t have to state a reason.
If they’re pushy enough to ask why (which they really shouldn’t), you can say it’s “for personal reasons.” There’s no legal requirement for them to go on a trip like this.
