I also had PCOS, and I was on a very low and slow protocol for stims, specifically to try to NOT damage the eggs, according to my RE. I know so many women with PCOS who were overstimulated and had 30-40 eggs retrieved, but then only one or two viable embryos.
I don't think a lot of people realize the relationship between PCOS and egg quality
My baseline, unstimulated, was 26 follicles. Again because my.team before Jubiz did NOT take my PCOS seriously and refused to treat it.
RE team did low and slow I ended up with 30 follicles, 14 mature eggs, 7 embryos day 5/6 but all were abnormal which was extremely disappointing. Previous cycle I had 3 day 6 and 1 was normal. So exact same outcome.
Have you ruled out other causes of infertility, such as tubal or male factor?
Yes, I've been through 3 years of "basic" tests by OBs and REs. But the RI put me through extensive testing that finally identified the blood clotting disorder that wveryone knew I had but they refused to treat it (along with PCOS). I have no issue getting pregnant, I can't stay pregnant.
Now that I'm in the know, I know how terrible the care is that I received.
I have an internet friend who was talked into donor eggs with an AFC of 30, age under 35, and a prior successful IVF baby. It turned out she had an occult uterine infection and her husband has poor sperm quality. They repeatedly blamed her egg quality with zero evidence. Like me, she has been to half a dozen IVF clinics. She's had over half a dozen FETs, including some of the DE embryos, and still has nothing to show for it. Her egg donor was a proven donor.
REs are basically used car salesmen. Very scummy and unethical group of people. Its all about money for them, and if you get pregnant after your first transfer, they don't make as much money compared to running you through the ringer.
Have you explored stuff like this?

I suspect you or your partner have an issue other than the PCOS that is preventing a healthy pregnancy.
We have lost all chromosomally normal babies, so before this point it wasn't poor egg quality causing my losses. However my PCOS has gotten much, much worse without treatment for the last 3 years.

Stop fighting with your RE. Fire him/her. Find a specialist. Or have the specialist make a plan for you and you may or may not tell your RE about.
As a general rule, once you're fighting with a doctor it is time to leave. I don't find anything good happening after that.
+1 I did one ER and only got one embryo (and OHSS, gift with purchase!), so I understand how much it sucks. But if you're creating 30 eggs and more of them could be viable with PCOS treatment, then you need to jump ship. It's easy to fall into the trap of just doing whatever they say because they're the experts, but oftentimes they're just sticking to whatever works for "most people" and not really you. If you're going to do another ER, find someone who will treat your PCOS before you try again.
I wish it were that simple. Truly I've have 5, 6 physicians decline to treat my PCOS before this specialist. Literally just say, go to someone else.
