I don't know that Lab is a sure bet. We have a DD there, but two separate coworkers of mine told me they applied there for middle school boys over the past several years and both were rejected with that profile (both mentioned severe ADHD, I'm not sure how severe the dyslexia was). I'm sure both kids would have done well there. I also suspect it could be slightly easier for girls because I think boys are over-represented.
Chelsea School and Diener are also options.

Severe ADHD rules out Siena. They old take mild/invisible cases.
The answer to the OP is Lab School. They are a bullseye for that profile.
