LOVE my handheld. We have them in every bathroom (separate if there is space) and use them all the time.

Hand held to clean the shower, clean the dog, etc. fixed mount for better water pressure. A 20 sqft shower is plenty big for both.

Yes, ours is probably smaller, looks like it replaced a tub, and I doubt it's 4 ft wide, maybe, anyway, we have both. Doesn't look or feel cluttered. Handheld and valve are on the side wall, there's a second bracket for it and the other controls on the end wall. Fixed shower head on the wall, rain head on the ceiling. They all get use, but a single head would be sufficient.

I don't think the shower is too small to have a separate hand held on side and showerhead on top. I don't like the showerhead being the hand held. It is really nice to have a separate one on the side. Use the diverter to go between the two. If the showerhead is a handheld in one - it has a long cord. It is just an aesthetic thing. Functionally separate hand held or combined with showerhead do the same thing.
