It’s 30 mins away, but call Falcon’s Landing. It’s a full retirement community only for military officers, so it’s nonprofit, but their skilled nursing facility accepts civilians if there is space. Light years beyond many we toured, even four star ones.

I was raised my a single mom who had me later in life and she now has advanced dementia. Put differently, I'm in a similar bind, also found the hospital social worker useless (and cruel), understand how stressful this is, am sending you lots of good energy.

My only suggestion is to make sure that the doctors are clear with you regarding precisely why they are recommending SNF and then trying to find workarounds to address those issues (recognizing that you might be choosing the least of the evils among the workarounds). For example, my mom's doctors recommended 24/7 supplemental care for my mom while she already in a facility with daily visits from me. After some pressing and explaining the services she already was receiving in assisted living, it boiled down to needing to ensure that she was agreeing to take her meds on time. I'm exhausted but I've taken that on myself, and it's been doable for a couple of months.

She's already had a terrible experience with a social worker.
I'm sorry OP, I don't have any specific recommendations. I will tell you that $17k a month is a STEAL--my parents were quoted twice that several years ago.

For skilled nursing? No. I am paying 15K a month for 24/7 skilled nursing in Rockville.

SNF should be covered by Medicare (for a period of time). Talk to the social worker at the hospital and find out which ones have bed available. You can research them on Medicare.gov for quality.
Once he’s done with either the nursing care/OT/PT or whatever he’s receiving at the SNF then you will need to figure out what he needs and what you can pay for:
1. Stay at the nursing facility (the S means skilled, it’s just the level of billing)
2 Find an assisted living that can handle his care. Sunrise and similar will take all types of care but the charge just keeps going up
3. Home based centers- example listed in a previous post
4. Go home and arrange for an attendant. Likely $25/hour -+ at least through an agency. Home attendants can be tricky because in our experience staffing was difficult.

If the case manager at the hospital isn’t explaining your options ask to talk to a patient advocate so you can better understand what your options are
