The "information at hand" is almost nothing. New poster but I defend those noting that possibly he had a medical issue that needed fixing. I"m not even clear that he and OP have met in person -- OP refers to "not wanting to do long distance" so are they just on Zoom or what? Pretty skimpy on details. I bet if he had a medical issue he wouldn't have told her but of course he can't hid the change to his nose.
So, OP, did he come right out and say he did it for looks or did he mention any other reason? And do you two actually meet in person or via screens?
As for "making up scenarios," PP above, how is it not merely "making up scenarios" when other posters insist he's vain? For all they actually know he's not, and getting this done wasn't a matter of vanity. They don't know any more than those saying maybe it's medical. You're preaching a double standard if you think it's fine for some PPs to insist he's vain but it's not fine for other PPs to suggest he might have had other reasons to do this.
None of us actually knows. Not sure even OP actually knows.

You being up a good point. If this surgery was had for purely cosmetic reasons that means he has a strong adherence to Western Caucasian Beauty standards. What about kids in the future?
There is a strong chance the kids would have his original nose . Would you want to raise kids with someone who thought part of them was ugly?

You are right. But women are expected to be a bit vain and value their preceived attractiveness. No one wants to date a vain man though.
