My guess is that Armie’s family has threatened to use their wealth to try to get sole custody. Has she even left The Caymans since the pandemic began? The FIL has a lot of political clout on the island and could make life very difficult for her.
I don’t know much about his family and their connection to the Caymans but if that’s the case it would be very foolish of her not to return with the kids to the US.
NP here. Do you think she has a getaway boat docked somewhere? She has to leave the island through the airports or other official channels and that’s not going to be possible if there’s a pending custody dispute.

If she takes him back and leads a separate life, she can protect her children. My mother basically had full and exclusive custody of my siblings and I while married to my father essentially in name only. He is an awful human being and divorcing him to leave us in his “care” half the time would have been disastrous.

It's truly awful. I am glad for you that you had your mother, PP.

And his family is very religious and would probably use said trust for them to stay together. She also has nothing going other than being his wife.

It helps their children by extension, if their dad is seen as just another human who battled his demons in rehab and came out a better man who could live a normal life, rather than a sociopath who abuses women.
This has to be the answer.
