Why was Kate the only member of the family with a weight issue? It got much worse after her father died but why was she an overweight child and teen? Does she have an anxiety disorder as also evidenced by her fear of swimming? They all treat her with kid gloves—why? Because she is the only girl?

DP here... Rebecca also had some issues with weight/eating as she was growing up. Not to the extent Kate did, but there was an episode about it. Kate did not have a healthy way to deal with her emotions, so she ate. When she was sad or depressed, she ate. Its a disorder and no one helped her. She should have gotten help as a young woman. At this point she needs surgery to get a start.

I actually think the writers have done this pretty well - obviously teasing that there could be something more to the ending than just Rebecca's death.

sadly likely she won’t even remember that she ever married Miguel. Most times with Alzheimer’s you remember early memories but not much past middle age.
She won’t even remember Jack or her kids or her grandchildren.
