This doesn't track because sex scenes are also just "make believe" but productions are expected to use an intimacy coordinator and treat actors respectfully when they are filmed because it's generally accepted that depicting such an intimate act requires vulnerability and exposure of the actors involved.
And the Florence Pugh birth scene mentioned upthread also talked about the same issues -- using a midwife consultant and an intimacy coordinator specifically because filming the scene was so intimate for Pugh (and Andrew Garfield) and they talked about how Pugh and Garfield didn't even have the makeup artists come in between takes so that they could maintain the intimacy of the scene -- they touched up one another's "sweat makeup" in between takes so that they could stay in the moment without interruption.
You make it sound like filming a scene where you are giving birth is no big deal, just another day of work, who cares. But that does not appear to be the standard attitude within the industry. And Baldoni keeps claiming he was really invested in "the female gaze" for this movie. If that was true, why didn't he make more of an effort to ensure the comfort of an actress while filming such a personal, intimate scene? It doesn't make sense, unless his whole feminist schtick is an act. Wouldn't he *want* Lively to be comfortable and feel supported in that scene?
