What would be the major problem with using headphones, though? Nobody cares what your kid is watching, they just don't want to hear it. If we are in a waiting room- whether my kid is watching a screen or I am reading him a book or my kid is being fussy... I DO NOT care about you. Not at all. If you cannot handle a noise in a public environment like that, you are the one with the problem. I would never take my kid outside unless there they were being a serious disruption.
A toddler throwing a fit and needing to watch a video/play a game at full volume is a serious disruption. It will get you escorted out of a decent number of places.
