I don't feel comfortable with handing over my medical record.
I feel like privacy has gone out the window and feel violated. Are other churches doing this registry, too?

Nope. My church (Greek Orthodox) trusts everyone is being truthful about being vaccinated when they come for services. However, I think your concern is justified and this is an intrusive request.

Wow. This is frightening OP. I don't like to see church and state mix like this. I'm a Christian and attend a Protestant church, so I don't know a lot about how synagogues are structured. Is there a lay committee of members who make policies like this for your synagogue? With whom can this be discussed and appealed?
