If she's the one who breaks it off then morally and ethically she should return the ring. Legally whether it is considered a conditional gift varies by state.

No, this is incorrect. Most states do not require a return of the ring.

Depends on the state but since she canceled in most states she would have to return it. In many states even if she did not cancel it she would still have to return it.
