I think your options are to continue in the nanny share or not. That's it. You don't get to decide what interventions the other family does, whether or not you are correct that there is an issue. It's not your place. At all.

Sounds like he needs an evaluation. maybe the nanny can record the child screaming, perhaps the parents aren't really hearing it the same way. or the docs when they go for an appointment.
