Update. I called the Board of elections. You do need to provide your license number and the last 4 digits of your social security number to request a mail in ballot.

They definitely want your license info— can’t remember about SS but I would just try it and see what happens.

Do not get your ballot emailed— that ballot can only be counted after it is hand processed as explained above.

Please do not print out the ballot yourself because if you do then it can’t be scanned— it will be opened by the elections office and someone will have to hand mark a ballot the same way you voted.

I just signed up to receive a mail-in ballot in Montgomery County. The process took about five minutes. They send them 30-45 days before the election, and you can mail them back or put them in a ballot box at a polling station (that's what I'm going to do, to bypass USPS and be sure it's received). You can also request they send you a link to print out the ballot yourself and turn it in. Could not have been easier.
