Not always. I have always done this and I am a full 180 from anxious/OCD. I do have ADHD, but don’t know if that’s related.
I was going to say the same - sounds more like a fidget/inattentiveness to me.

Hi. It me! (But seriously) it’s bad once the focus moves to skin. Try to Kew their hands busy with something else, fidget spinner... really anything.

Yes I have one of these, my four year old. I call her the Destructor. She’s a tinkerer and can’t help herself but pick at things and pick them apart.

Does anyone else have a kid like this? My 6 yo has a compulsion to pick things -- like if there is a small hole in his pants, he can't stop picking at in until it's huge. Or a loose sticker on a book -- can't leave it alone. Tiny tear on the library wrapping on a book? He's going to rip it bigger. There was a small loose seam on one of our dining room chairs, he seems determined to make the damage worse.
He's otherwise well-behaved, but this habit is so annoying.
