We got a simple form from our estate attorney and it includes a section for a witness to sign, attesting that they saw you sign it. The witness can’t be related to you or the people you designate on the form.

My MIL and FIL will be watching our two children while my DH and I take a trip for a few days that will be out of the country (it's a big birthday and been a rough year and in laws are willing. In laws already regularly watch kids).

We will be expanding our cell phone service so that they can reach us, but I'm also wondering about a temporary medical consent form or something like that in case a kid gets sick and they do have trouble reaching us. Is there an easy enough way to print out a form online for this or do I need to go through a lawyer?
