Location of employer. Like stated above, it's going to depend on the HQ. Husband works in MD, but his insurance still doesn't have fertility coverage. There's no waiting period, but if you were to have infertility coverage, you'll most likely have certain criteria to be met for IVF, which could include so many months of trying.

Also, the mandate does not apply to federal health insurance.

More specifically, I think it also depends on the size of your employer (more than 50 employees) and whether their HQ are located in MD.

s the maryland mandated coverage of ivf based on the location of the employer? or the residence of the employee? also, is there a waiting period?
