This is pretty much what I was going to say. I would probably hate an unflattering picture of myself (which is most pictures, to be honest) and wish you hadn't sent it, but if you write a lovely note with it and say how much you love the picture and how you can see their connection with each other and how happy that makes you as a mother, than that might soften the blow for me.

+1 for both of these.

This. Don’t apologize for the bad angle or anything like that.

You're overthinking it. Send the picture already.

This is such a sad post. Maybe she won't want to display it in her living room, but I doubt that she wouldn't like to see the photo just because it isn't perfectly flattering.

No, I tried that.

And grandma isn’t old! She’s a young 65! (Grandpa remarried well lol)

Is she vain? If so, don't send it. Otherwise, send it and don't worry.

Not really, but she mentions her weight gain in a self deprecating way occasionally.

Just send it. Don't overthink. If she doesn't want to frame it and display it because of the angle, she may still treasure it. Let her worry about whether she thinks it isn't flattering.

Would cropping help?

Send the photo. Us old folks are at lot less concerned with our physical appearance than we were when we were young. We know what we look like. It sounds like a lovely photo and grandma should be happy to get it. I am sure that she will appreciate the gesture.

I caught a great photo of DD and her step-grandma playing the piano together; something they do whenever we visit. It was taken from behind—they are gazing at each other and look happy. However, and I don’t know if it was the angle or what, but grandma looks a little “wide” from behind, if you know what I mean. As a woman, it’s a photo I wouldn’t want to see of myself, to be honest. But the photo just captures this special bond they have together. I was going to print it and send it, but maybe I shouldn’t?

Ladies? What do you think?
