For IVF coverage pay privately on the market place ~$450/month. Aetna negotiated rate that I am aware of through SGF is for FET at $1850. not sure what the rate for fresh IVF is if there is even a negotiated rate for that.
IVF meds would be a complete OOP since no coverage for that as well. ~3-8K/cycle. FET meds OOP run you ~$200 or less. But you may get lucky for insurance to cover progesterone or estrace as they both are used in pregnancy related cases.
Can you pay privately on the market in DC for IVF coverage (I haven’t seen anything), or does this only apply to Maryland residents?
Since DC isn't considered a state and is federal territory there probably isn't any plan on the marketplace in DC that covers IVF. The coverage for IVF is mandated at the state level hence MD is a mandated state there is some IVF coverage.

The change with insurance has been recent - within the last year or two. There is not a single Federal plan that offers any sort of coverage for IVF. The "negotiated rate" with Aetna that is often referred to on these boards is, to my knowledge, only offered through SGF. But it isn't actual coverage through Aetna.

IVF was a few years ago for me but Aetna did cover a good chunk of it. There were PLENTY of out of pocket costs too but once we did all the paperwork (ARGHHH!!) it went through.
