The financial folks at your clinic may be able to tell you their past experience with your particular insurance. But past practice is no guarantee of current practice. I had coverage with a different insurance company, and my clinic previously found that that company would approve IVF after three failed IUI’s. But then the company changed its internal practice to require 6 IUI’s (no change in the coverage language, just a changevto internal practice) so we ended up paying for IVF OOP.

I remember our nurse telling us we could move on to IUI with injectables after our 3rd failed IUI (with just clomid) OR we could move on to IVF. Said insurance would cover 3 more rounds.but we just moved on.

They required us to have 3 before moving to IVF. However, be warned that despite that-arranging the medications for example through their only approved supplier and all the other details was extremely stressful.
We ended up moving on to private.

That language seems to indicate they will require you to undergo the 6 IUIs that they provide coverage for. If you've already gone through (unsuccessful) IUIs, perhaps they'll accept documentation indicating such and you can move on to IVF. You should contact them directly and ask for specifics in writing. OP here again. the plan covers up to 6 IUIs.

We are considering switching insurance. The plan we are considering switching to says “i. The Member has been not been able to attain a successful pregnancy
through a less costly infertility treatment for which coverage is available.”. Would medicated, times intercourse count?
