Document incidences carefully before you bring them to HR. Dates, specifics about what was said. Maybe include detail about the contexts (in a meeting, in a casual hallway conversation, in a one-on-one meeting).

Perhaps others more knowledgeable about the legal situation might disagree, but if I were doing this, I'd pull in at least three carefully documented incidents and present them in writing to HR (by email, so there is a record of the transaction).

Yes it is gross and inappropriate but definitely do NOT do this. This is tough because normally I'd say go to the person and talk to them first but as your boss he can make or break your career at the company so I'd just go to HR and as for the complaint to be kept confidential.

Report it to HR, if that feels like a safe route to you.

It doesn't have to be directed at anyone, if it makes you uncomfortable you should report to HR. I'm pretty sure our harassment training (assuming your employer has one) covered this.

Boss makes frequent references to him being naked (in stories about his life), references sex acts, is somewhat crude in his language. But it's not really directed at anyone. What would you do? He is a good boss otherwise.
