Totally second the idea of looking everywhere in the kid's diet to substitute calorie dense foods for low-cal versions (the exact opposite of what we adults do).

Trader Joes has many fairly healthy cereals that are >250 cal. per cup, compared to Cheerios 100 cal per cup.

Whole milk and full fat yogurt rather than skim. Lots of butter.

Many kids have trouble drinking protein shakes. Consider a whole milk and greek yogurt smoothie instead --- 30g protein, at least 400 cal and easier to swallow. OR Gatorade protein shakes are 20g. protein and 250 cal.

Guacamole is good.

larger portions? pasta? I'd try to avoid too much sugar and fat because that's just bad for their hearts, like it is for adults.

I don't see why this question comes up so often? Why are the moms on here unfamiliar with food - is everyone on a permanent juice cleanse?

Look at the places where you can substitute higher fat versions of things. Use more butter.

My son's nutritionist said to avoid cheerios because they displace foods higher in fat.

I always found a daily malted did the trick.

I also found that you shouldn't worry about nutrition so much when your goal is stabilization. Once you've stopped the weight decline and are feeling better, then you can worry about making sure things are more balanced.

You could also have them drink meal replacement shakes like Boost. Not as meal replacements, just as drinks. 2-3 a day would add 600+ calories easily.
