No, once you've turned in the medical form for K (or whenever they first register when entering the system), then they're good until 6th grade or until the next round of vaccines. No medical form is needed for 1st grade, 2nd grade, etc.

But in our schools (ES, MS and HS) they always email a copy with back to school paperwork and/or open house information, make parents fill it out at the back to school open house before the kid can get the schedule for MS and HS, and put a copy in the first Tuesday (Wednesday, etc) folder, etc., etc. Highly likely that you will have the form sent/ handed to you at least 3 times before the end of the first week of school.

Help. I've gone to the site but cannot seem to find what I'm looking for and for some reason I have totally blanked out on this. Does FCPS require school medical authorization forms every year? My child has an allergy so I am so focused on getting that paperwork every year for school and camp that I have totally spaced on what my non-allergy child's requirements are for paperwork.
