Another thread suggested this may happen when you don't submit a lot of embryos for testing. This was the case for me, so maybe this is the reason. I'm wondering, though, what happens if you keep sending in embryos. At what point as you get closer to 12 do they decide to go ahead and collect the additional $1000?
I paid $2000 for the testing (up to 12 embryos) plus the clinic charged a $1500 biopsy fee on top of that... Maybe they originally quotes you for the testing fee plus the biopsy fee?

I had 15 tested and had to pay a "per embryo" rate for anything over 12 embryos.
To respond to 19:47, my understanding is that you are not charged again; I think the fee is for 12 embryos regardless of the number of cycles. I would definitely check with your financial coordinator, though.

I am OP. I tested 5 over several cycles. One was "no result," 3 were abnormal, and one was normal.
I have never been told that the 2K only covers you until you get a normal. My normal didn't take. So now I have to pay all over again even though I have only had 5 tested? I don't think it says that in my paperwork.

I sent in biopsies from 8 embryos and was told it was 2K for up to ten at FEC. There is not a discount if you submit only three but they'll charge you more if you submit 11.

Same happened to me. I sent in 3 biopsies and only was charged 2k. I never sent in more (it's up to 12 or until you get a normal, whichever happens first)
