+1 Virginia camps have always accepted the DC form.

Can you speak for all VA camps? I just had to use a separate Va form for camp.

Received a letter from my kids nurse just last week informing us about my kids physical form expiring this month.

No they don't. I've had kids in schools in five different states. They did not all require yearly health forms from the doctor. They all did require proof of immunizations on their state forms when we registered as a new student.

+1 This is my experience from living in different states as well. Our previous doctor's office didn't charge to complete the form either.

This is not common knowledge, if it is true. Neither our MCPS school's health employee nor our doctor, who has been filling out forms for kids for years, are under the impression that the requirement is yearly. However it's common knowledge that DC does have this requirement.

FORM: Universal HealthCertificate

Description: Two-page form, and two-page instructions for your medical provider

Required: Students enrolling in all grades (PK3-12th).

NOTES: Have your child’s physician or nurse practitioner complete the Universal Health Certificate.

The Universal Health Certificate must document immunizations, tuberculosis assessment and physical exam completed within 365 days before the start of school. Every child less than six years of age must be tested twice for blood lead poisoning. Testing must be completed, regardless of exposure risk, and documented on Universal Health Certificate.

No they don't. I've had kids in schools in five different states. They did not all require yearly health forms from the doctor. They all did require proof of immunizations on their state forms when we registered as a new student.

When I signed my son up for K they said he won't need them again.

Read the forms - they say otherwise (must be no more than 365 days old). But then again, you may get the same clueless person when you enroll next year.

I just make copies of the original. I'll also scan them to my email so can access them and/or forward them when needed.

All schools require them yearly.

It just seems so excessive. Some camps and aftercare require it as well. Some camps are different state forms so can't re-use. Doctors charge $25+ a pop. With 2-3 kids, you're talking about upward of $200/year just on forms.
