OP here. Thanks all! I had no idea about titres. I also do not mind getting all the shots over again if I have to do so. I just wasn't sure if any of these shots had to be given only as a child or what not. If urgent care can do this, that will be great but if not, time to do find a doctor. Also, no idea about the doctor who did my shots. I remember it was an in home doctor and he retired when I was still a child. My parents do not have records

I think that a PP brought up religion, not the OP.

OP who gave you your shots? Only you can tell us the answer to that. For most people, it was a pediatrician and if that was the case for you, you should request the records from that pediatrician.

It's not uncommon to receive boosters as an adult, if you have expired shots, or some of your records were lost and titres show you need them. Urgent care can give you most shots you need.

My advice is that once you have resolved this, make a record and store it in the same place as you store your birth certificate, your official transcripts, Social Security card, your parents' will... you get the idea.

This happened to me with grad school. First, check with your pediatrician from childhood to see if they have any records. That might work. I was able to get some that way but not all. Ultimately, I had to get a bunch over again that were required in my state of attendance. Different states have different requirements. I was able to get them at student health.

PP here. I misread- was your religion part of you not getting shots? If not, tell them the truth and see what they say.

Perhaps you can go have titres drawn. The results will indicate of you still have immunity to something (i.e., measles). If not, you can get a booster shot of whatever you need.

Don't be surprised to learn you are missing shots and need some. This happened to me.

I am attending college and I need to fill out a medical form, along with my doctor. Listed needs to be records for when I received certain shots. The problem is, I do not have these records, and my doctor does not either. I actually do not have a doctor. I am 30, I see an obgyn, and if i get randomly sick, I go to urgent care. So, how or where do I go for my shot records? I have no clue how to fill out this form.
