Ugh, my name at birth is not what my name was at one year old, so I wonder if I need my birth certificate, court paperwork, AND marriage certificate!

Thanks. I wondered the same, since it only says Last name (at parents birth) I thought it would current first and middle and then maiden last name.

It's wrong. Your child's birth certificate lists YOUR maiden name. Look at it.

Funny, we just submitted for our kids last week, and I now wonder if we messed it up. I assumed my name at the time of my child's birth.

I have a situation where I was born with a different last name, but at 4 my name was legally changed. So all my maiden name stuff is NOT what my birth certificate would be. What would I do for that? Also, my daughter's birth father is NOT on her birth certificate although I have documentation that gives me FULL EVERYTHING. Do I bother putting his info on there? He has nothing to do with her/us.

We recently renewed my daughter's passport, and the woman processing it thought the photo was too light. She asked if we wanted them to retake it for $12 or take a chance and send it in. I asked the repercussions of sending it in and the passport agency not accepting the photo. And she said that they would still process the passport and just send us an email for the part that is missing. In other words, it wouldn't mean starting over from scratch, but it could delay it a bit while we got them the new photo. It turned out that the photo was fine, so I cannot confirm.

Sorry to revive an old thread -- if you *do* make such a mistake (e.g. use mother's married last name rather than the maiden name on the form) does the passport office still process the passport or does it return it to you so that you have to start over?

I did this incorrectly the first time I applied. PP is correct, it's your given name at your birth.

I just got my passport with my married name and did submit a copy of my marriage license. Same for when I applied for my son. I submitted my marriage license and his birth certificate.

Thanks, PP. I'm wondering if I need to bring a copy of my marriage license, since my current passport and driver's license have my married name.

I don't remember this being on the form when we applied for his first passport 5 years ago.

+1. They want the name you were given when you were born, that would match up with your birth records for verification if needed.

I'm filling out form DS-11 to renew my son's passport. In section 10, it asks the following:

Mother/Father/Parent- First & Middle Name ********************** Last Name (at Parent's Birth)


Here's my conundrum. Say my maiden name is Jennifer Ann Smith, but my married name is Jennifer Smith Jones. (This is not a hyphenated last name. Smith is now my legal middle name.) Do I write:


Mother/Father/Parent- First & Middle Name ********************* Last Name (at Parent's Birth)

Jennifer Ann ********************************************** Smith

or

Jennifer Smith ********************************************* Smith

????
