OP here: I have an aversion to putting it over the fireplace, but like a PP mentioned, it is open concept. One wall is all windows and the wall with the mantle has windows on either side, so the TV would not only be off center, but block windows. The third wall is the side of a staircase and a traffic area that will not work. The "back" of the family room area is the kitchen.

There are mounts that position the tv downwards at an angle.
Unless you are 90 years old and suffer from a spine injury from ww2 you'll be fine.
Just make sure that the mantel above the fireplace directs the heat forward

we have that and it is totally fine and who cares what others think about it. it is what you want and if they buy your house after you then they can put it where they want.
the size of the tv is on a diagonal line not straight across. cut out a piece of paper the size you desire and put it up there and get a feel for it. use dark paper. also get a good av guy to run the wires etc in the wall.

Try to find another space. That's two focal points competing for attention in the same space. It never works.

We are in a similar position and decided to use the landing area between floors for a small couch and TV rather than put the TV in the living room. But we have a strong aversion to having the TV in the main living area.

I don't like TVs over fireplaces, but if I had to choose, it would be smaller than the mantle.

The only other option right now is the basement (which we are not furnishing just yet), so the living room (which is combined with the kitchen) is where it has to be.

Wouldn't that be two competing elements?
A fireplace/mantel should be a centerpiece and focal point to the room.
The TV would be a competing focal point.
How about putting the TV in another room? You don't want it to look too cramped. The TV would compete with the mantel.
