Biopsy (fee goes to SG) 1500
Actual PGS (fee goes directly to genetics lab) 2500-3000, depending which lab you use.
ADDITIONALLY, they use ICSI with PGS so that's another 1800-2000
AND for freezing there is a separate fee as well.
After all said and done it's about 8 (eight) grand. Unless you have a specific genetic disorder, insurance considers it experimental and does not pay a zip.
If you expect to have embryos from different cycles - the cost depends depends. If you freeze all and bank them over more than one cycle - yes, same fee, but depending on the number there might be an extra "per unit" fee (over 12 believe). If you want a fresh cycle with overnight PGS you have to pay every time. They should give you detailed and specific info.

Some people are saying $2500, but the lab has changed as of about a month ago. Perhaps they are still working with both labs in which case you could get the $2500 fee. If you are in Shared Risk, the SG fee goes up to $4000. So I paid a total of $7000 for everything. Hope this helps!
