Our school uses a large closet as a used uniform "store". A student or parent can go into the store at any time to "shop". The clothes are donated by parents whose kids have outgrown the uniforms or moved away. They use an honor system (exchange, or pay a nominal fee).

My school teamed up with a consignment store. I haven't used it. Most of my kids clothes are pretty well worn by the end of the year. There's usually a couple items that weren't worn much and I give those to friends that have smaller kids in the school.

My DD's school does a swap that works fine. It probably isn't the absolutely BEST method, but it is okay. On the day of Back to School Orientation (a week before school begins), there is a swap in the gym where orientation will be held. The doors open about 30 minutes early; uniforms are available for swap on a first-come, first serve basis. If you bring a piece, you can swap for any piece available in your child's size. There are volunteers there to take your pieces and guide you to the section containing your child's size(s). Of course, if you are first in line, you get the best picks.
