I believe you do need a student ID number in order to register for Extended Day. Just fyi, you can go ahead and send in the rest of the packet without the health form. My DD is a late summer birthday, and the registrar said it was fine to just drop off the health form when it was completed in August.

Speaking of which, more on topic -- my older kid has had wonderful experiences with Summer Extended Day! The 3-week extended day program at ATS that takes place during the elementary enrichment programs (Global Village and Summer Laureate) is awesome. It's really more like a summer camp, with special activities and events and even a couple of pool visits and field trips. Way above and beyond what I expected.
