You learned a lesson. I was the poster thinking about taking a job with three people in two years. I say leave it off. If someone finds out or asks, keep a reply short and simple. I feel sorry for the next schnook who takes the job.

I also vote for leaving it off. If the gap comes up subsequently explain the great things you did in the role. Briefly and openly describe the unstable, high turnover environment that led to your separation.

I'd leave it off, unless this is a background check type job where they ask for all of your previous employers. Recently observed a hiring process in my company and I was surprised how lackadaisical they were about calling references, even when it was kind of clear from circumstances that the candidate had been let go from a job earlier. Nobody seemed to really care to dig that up once they decided they liked the candidate and were satisfied with her other, good references. So I'd say - leave it off, and just put on the good ones!
