OP, I get you. My pet peeve is when I order hot fudge and instead get cold, thin, fake tasting chocolate sauce. No, it is not even close to an acceptable substitute.

This is terrible. I have instructed my husband, who is a quicker eater than I, to hold onto his plate!

OP again - yes! This and when I don't get a clean fork for the next course. Even in a more casual restaurant, but especially in a nice restaurant.

I KNOW this is ridiculous, but my giant pet peeve is when a menu says "caramelized onions" and they're really just grilled or sauteed. I'm addicted to GOOD caramelized onions, and I think 90% of the time they get it wrong.
