I'm thinking there's got to be "women's" underwear for men that solves this problem for cross-dressers. Though again, does that defeat the purpose? Find cross-dressing forums for advice.

Sizing will be hard, because women's underwear is not designed to accommodate your genitals. They all get really small in the crotch area.

You might like a boy short, but that will defeat the purpose, no?

Probably a large or it extra large. Try a size 8, if they are 5,6,7,8.

Some panties are super stretchy. Shimera and Calvin Klein Seamless or Second Skin lines are made from the stretchy material.

If you get something with stretch, the sizing is less precise, and they're more comfortable.

To add to that suggestion, choose a brand and check out that brand's size charts. Many brands use sizing that has little correlation to typical pant/skirt sizes...I'm a woman and I get confused when buying underwear sometimes.
