None of our purchases have been unusual lately, and because we're not getting out much lately due to baby, we have possession of our card most of the time. The only recent occasion when we've handed it over was last Saturday at Iron Gate (dinner). DH just signed up with Uber this week. Could either of these things lead to our stolen cc number?

Yes. Anytime you use your card ANYWHERE to purchase ANYTHING - online or in person, there's a possibility of theft. Its just a 16 digit code, nothing more.
